The CSS is responsible for coordinating the care delivered by the CNA, Homemaker and PCA staff. The CSS is responsible for staffing efficiently in order to meet the client’s needs and maintain effective scheduling practices for the Agency.

One year of Home Health experience preferred.

Receives requests for services.
Participates with the physician, referral sources, Administrator, Clinical Supervisor and professional staff in coordinating home care services.
Communicates with the client care manager to coordinate professional and ancillary services.
Works cooperatively with the Clinical Supervisor and/or Administrator on staffing, referral sources, physician and client concerns/needs.
Communicates effectively with clients and families regarding physician order changes related to schedule changes and staffing changes as requested.
Accurately verifies visits made with schedule in software system. Maintains schedule of both client and employee in software system.
Fills out Missed visit form as indicated.
Involved in On-Call rotation.
Receives night, weekend and holiday requests for nursing services. Assigns staff as appropriate.
Handles any rescheduling needs for workers absent when on-call. Ensures services are scheduled for clients with priority need.
Receives report related to on-call services from staff. Communicates concerns to nurse on call and/or Administrator on-call.
Communicates client changes in timely manner to the client care manager, case manager or support services manager..
Assists in maintaining Referral, Hospitalization and Discharge boards.
Receives reports from staff as requested. Communicates concerns to client care manager.
Other duties as assigned.
